To ensure your preferences are respected, and to comply with our SMS provider’s guidelines and applicable data-protection rules, please review the following terms and complete our opt-in process.

What you are consenting to
By completing the opt-in form, you confirm that you wish to receive SMS messages from us regarding [brief description: e.g., product updates, service alerts, promotions] (the “Messages”). You understand that:

• Message and data rates may apply.

• The frequency of Messages will be [insert expected frequency or range, e.g., “up to 4 messages per month”] (or: “varies depending on your preferences and our program”).

• You can withdraw your consent at any time (see “How to opt out” below).

• Your telephone number will be used by us (and our service provider) for the purpose of sending Messages, and will not be shared with third-parties for unrelated marketing without your further explicit consent (unless required by law).

How to opt-in

1. Download the PDF opt-in form by clicking the button below.

2. Print the form and fill in your details (mobile number, name, date, signature).

3. Submit the completed form to support@flex-metrics.com.

4. Once we receive the completed form, we will send you a confirmation SMS verifying your enrolment and providing the first Message.

5. After your confirmation, you will begin receiving SMS updates tailored to your preferences.

How to opt-out / withdraw consent
If you no longer wish to receive SMS messages from us, you can:

• Reply STOP to any Message you receive, and you will be unsubscribed immediately (or as soon as technically feasible).

• Reply HELP for assistance or more information.

• Or contact us via support@flex-metrics.com to request removal of your number.
  Once unsubscribed, we will not send any further SMS messages to that number unless you opt-in again.

Your rights under EU data-protection law (GDPR)
If you are located in the European Economic Area (EEA) or the UK:
